Medicare GLP-1 Bridge Program 2026: $50/Month Weight Loss Drugs Explained
Updated September 2026 · 8 min read
For millions of Medicare beneficiaries who have wanted access to weight loss medications like Wegovy and Zepbound but could not afford the $800-$1,350 monthly retail price, the new Medicare GLP-1 Bridge program offers a significant breakthrough: a flat $50 per month copay. Here is everything you need to know about eligibility, covered drugs, limitations, and how it affects your Medicare costs.
The GLP-1 Bridge requires BMI of 35 or higher.
What the Program Covers
- Covered drugs: Wegovy (semaglutide) and Zepbound (tirzepatide) — for weight management only
- NOT covered: Ozempic (already covered under Part D for diabetes), Mounjaro (diabetes indication)
- Cost: flat $50 per month copay regardless of your usual Medicare coverage phases
- Duration: July 2026 through 2027 (temporary demonstration program)
Eligibility Requirements
- Enrolled in a Medicare Part D plan or Medicare Advantage plan with drug coverage
- BMI of 35 or higher — check your BMI here
- Taking the GLP-1 medication specifically for weight management
- Have NOT filled a GLP-1 prescription under Part D earlier in 2026
- Do NOT have a diagnosis (Type 2 diabetes, obstructive sleep apnea, etc.) that already qualifies you for GLP-1 coverage under standard Part D
- Doctor must submit prior authorization to a central CMS processor
Important Limitations
The $50 copay does NOT count toward your deductible or out-of-pocket cap. This is a critical detail many people miss. Under normal Part D benefits, your spending counts toward your annual deductible and the $2,100 out-of-pocket cap (for 2026). The Bridge program copay sits outside your regular benefits — it does not bring you closer to catastrophic coverage.
This means if you are also taking expensive medications under Part D, your Bridge spending is completely separate from your progress toward the out-of-pocket cap.
Cost Comparison: Bridge vs Other Options
- Medicare GLP-1 Bridge: $50/month (if eligible)
- Standard Part D (for diabetes): varies by plan, but $2,100 annual OOP cap applies
- Retail without insurance: $800-$1,350/month
- Manufacturer self-pay (NovoCare): $349/month for Wegovy
- GoodRx coupons: starting around $199/month
- Manufacturer patient assistance: potentially $0-$25/month if income under 400% FPL

Looking Ahead: 2027 Negotiated Prices
CMS selected Ozempic, Rybelsus, and Wegovy for the second round of Medicare drug price negotiations. The negotiated price of $274 for a 30-day supply takes effect in 2027. This will significantly reduce costs for Medicare beneficiaries using these drugs under standard Part D, potentially making the Bridge program less necessary for some enrollees.
