Medicare Part B Premium & IRMAA Calculator

Your 2026 or 2027 Part B premium and income-related surcharge (IRMAA)

Medicare charges higher Part B and Part D premiums when income is above certain limits. The income used is your modified adjusted gross income (MAGI) from two years earlier: 2025 income for 2027 premiums. The 2027 standard premium is the Medicare Trustees projection until CMS announces the official figure.

2027 Medicare Part B Premium and IRMAA (Projected)

The 2026 Medicare Trustees Report projects a $209.50 standard Part B premium for 2027 (up $6.60) and a $292 deductible (up $9). Each IRMAA level is set by law as a fixed multiple of the standard premium, which gives these projected totals. 2027 premiums are based on 2025 MAGI; the income thresholds are adjusted for inflation and will be confirmed when CMS announces the official figures, usually in November.

The IRMAA Cliff

IRMAA brackets are cliffs, not gradual increases. A single person with 2024 MAGI of $109,000 pays the standard $202.90 in 2026; at $109,001 they pay $284.10 plus $14.50 for Part D, about $1,148 more for the year because of one extra dollar. For a married couple both on Medicare, crossing the first joint threshold costs about $2,297. That is why the calculator shows how close you are to each edge.

Ways to Lower or Avoid IRMAA

  • Plan income two years ahead. Spread Roth conversions and large capital gains over several years to stay under a threshold.
  • Use qualified charitable distributions from an IRA after age 70½; they count toward required minimum distributions but are not included in MAGI.
  • Contribute to pre-tax accounts while you are still working, including an HSA before you enroll in Medicare.
  • Appeal after a life-changing event such as retirement or the death of a spouse, using Form SSA-44.

Frequently Asked Questions

What is IRMAA?

IRMAA (income-related monthly adjustment amount) is an extra charge added to Medicare Part B and Part D premiums for people with higher incomes. It is based on your modified adjusted gross income (MAGI) from your tax return two years earlier, so 2027 premiums are based on 2025 income.

What are the 2026 IRMAA brackets?

For 2026, there is no IRMAA if your 2024 MAGI was $109,000 or less (single) or $218,000 or less (married filing jointly). Above that, the total Part B premium rises in five steps from $284.10 to $689.90 a month, and a Part D surcharge of $14.50 to $91.00 a month is added.

What will the Medicare Part B premium be in 2027?

The 2026 Medicare Trustees Report projects a standard Part B premium of $209.50 a month for 2027, up from $202.90 in 2026, and a deductible of $292, up from $283. CMS usually announces the official 2027 figures in November; this page will be updated when it does.

How is MAGI calculated for IRMAA?

For IRMAA, MAGI is your adjusted gross income (line 11 of Form 1040) plus tax-exempt interest income. Roth conversions, capital gains, and required minimum distributions all raise it, which is why a one-time income spike can trigger IRMAA two years later.

Can I appeal IRMAA?

Yes. If your income dropped because of a life-changing event (marriage, divorce, death of a spouse, stopping or reducing work, loss of a pension, or loss of income-producing property), you can ask Social Security to use more recent income by filing Form SSA-44. You can also request a reconsideration if the tax information Social Security used was wrong.

Does IRMAA apply to both spouses?

Yes, if both of you are on Medicare. Each spouse pays the IRMAA amount set by your joint income, so a married couple pays the surcharge twice. Choose "Both spouses" in the calculator to see the household total.
