Medicare Part B Premium 2027: What You Will Pay

Published November 2, 2026 ยท Will be updated when CMS announces official 2027 figures ยท 6 min read

Quick answer

The standard Medicare Part B premium is projected to be $209.50 a month in 2027, up from $202.90 in 2026, with a $292 deductible (up from $283). Higher-income enrollees pay more through IRMAA, projected at up to $712.30 a month. These are Medicare Trustees projections; CMS normally confirms the official numbers in November.

2026 vs 2027 at a Glance

2026 (official)2027 (projected)
Standard Part B premium (monthly)$202.90$209.50
Part B deductible (yearly)$283$292
Highest IRMAA Part B premium (monthly)$689.90$712.30
Income year used for IRMAA20242025

The projected $6.60 increase is much smaller than the $17.90 jump from 2025 to 2026. The Trustees' 2025 report had projected an even higher 2027 premium; the 2026 report revised it down.

What the Increase Means for Your Social Security Check

If your Part B premium is deducted from Social Security, the new premium comes out of your January 2027 payment, the same month your cost-of-living adjustment (COLA) takes effect. Your notice from Social Security in December shows both: the raise and the new premium. For most people the $6.60 increase uses only a small part of the raise.

A hold harmless rule also protects most beneficiaries: the standard Part B increase cannot reduce your net Social Security payment below the prior year's. It does not apply if you pay IRMAA, are new to Medicare in 2027, or pay premiums directly rather than from Social Security.

IRMAA: Higher Premiums for Higher Incomes

If your 2025 modified adjusted gross income (MAGI) is above the IRMAA thresholds, you pay more for both Part B and Part D in 2027. For 2026 the first threshold is $109,000 for a single filer and $218,000 for a married couple filing jointly, and 2027's thresholds will be adjusted for inflation. By law, each IRMAA level is a fixed multiple of the standard premium, so the projected 2027 Part B totals are $293.30, $419.00, $544.70, $670.40, and $712.30 a month, plus a Part D surcharge.

Budgeting for 2027

  • Standard premium: $209.50 ร— 12 = about $2,514 a year per person (projected).
  • Deductible: plan for $292 before Part B starts sharing costs, then 20% coinsurance on most services unless you have a Medigap plan or Medicare Advantage.
  • Married couple both on Medicare: about $5,028 a year in standard Part B premiums alone.
  • Drug coverage: Part D out-of-pocket costs are capped each year ($2,100 in 2026), on top of your plan premium.

Ways to Keep Your Premium Down

  • Watch your income two years ahead. A Roth conversion, home sale, or large capital gain in 2025 can trigger IRMAA in 2027.
  • Appeal if your income dropped. Retirement, divorce, or the death of a spouse are life-changing events: file Form SSA-44 with Social Security to use your newer, lower income.
  • Check Medicare Savings Programs. If your income and assets are low, your state may pay your Part B premium for you.
  • Enroll on time. The Part B late-enrollment penalty adds 10% to your premium for each full 12 months you could have had Part B but didn't, usually for life.

Frequently Asked Questions

What is the Medicare Part B premium for 2027?

The official 2027 figure had not been announced as of November 2, 2026. The 2026 Medicare Trustees Report projects a standard premium of $209.50 a month, up $6.60 from $202.90 in 2026. CMS typically announces the final number in November.

What is the 2027 Part B deductible?

Projected at $292 for the year, up from $283 in 2026. After you meet it, you generally pay 20% of the Medicare-approved amount for most Part B services.

Is the Part B premium taken out of Social Security?

Yes, if you receive Social Security benefits the premium is usually deducted from your monthly payment. A "hold harmless" rule stops the standard Part B increase from lowering most people's net Social Security check below the previous year, but it does not protect people who pay IRMAA, new enrollees, or people whose premiums are not deducted from Social Security.

Who pays more than the standard Part B premium?

People whose modified adjusted gross income from two years earlier is above the IRMAA thresholds: for 2026, more than $109,000 single or $218,000 married filing jointly. For 2027 premiums, Medicare uses your 2025 income.

When will the 2027 Medicare premiums be announced?

CMS usually announces next year's Part A and Part B premiums, deductibles, and IRMAA brackets in early to mid November. We will update this article and our IRMAA calculator when the official numbers are published.
